New User Setup Request Form

Explanation of fields:

  • Ticket Number:

    This is the number that will be assigned to you upon initial contact to onboard a new user. If you do not have a ticket yet, just enter “New Request” in field and we will create and email to you.

  • Requestor Business Name:

    The name of your company.

  • Requestor Name:

    Your name or the person you would like to document as requesting the new-user on boarding.

  • Requestor Email:

    Your email or the email of the documented person requesting the new-user on boarding.

  • Requestor Job Title:

    Your job title or the job title of the documented person requesting the new-user on boarding.

  • Requestor Contact Number:

    Your contact number or the contact number of the documented person requesting the new-user on boarding.

  • Email to send temporary password to:

    This email address will receive the temporary password that we will generate in your Microsoft 365 Organization. It will need to be changed by the end user prior to using on any software or apps. They can change it by opening up a browser and going to office.com. In the login fields, they will input their email and temporary password. Upon logging in, they will be prompted to provide a new password that meets company policy complexity.

  • End User Site Location Name:

    If you have multiple locations, please indicate which location name the user will be staffed at.

  • End User Office Address:

    This is the address of the end user primary office location.

  • End User New Email:

    End user corporate email. If the end user does not have a corporate email address set-up yet, please indicate what it needs to be.

  • End User Full Name:

    End User full name.

  • End User Job Title:

    End User Job Title. This is necessary to properly record into your 365 Organization for security, organizational chart and policy administering. Will also be used for the corporate email signature.

  • End User Department:

    End User Department. This is necessary to properly record into your 365 Organization for security, organizational chart and policy administering. Will also be used for the corporate email signature.

  • End User Office Phone:

    This is the phone number and extension of the end user primary office location.

  • End User Cell Phone:

    This will be used for the Microsoft 365 Password Self Service reset tool authentication.

  • End User Reports to:

    Who manages or supervises New User.

  • End User Assigned PC:

    This is the computer name of the assigned PC. You can easily get by Start-Run-Type CMD. Then at cursor, type “Hostname” without the quotes. This will be used to assign remote agent, security programs and customization/inclusion/assigning to in Microsoft Azure.

  • End User Shared Emails:

    A shared email is a type of user mailbox that doesn’t have its own username and password. As a result, users can’t log into them directly. To access a shared mailbox, users must first be granted Send As or Full Access permissions to the mailbox. Once that’s done, users sign into their own mailboxes and then access the shared mailbox by adding it to their Outlook profile. What emails does this person need to be a pat of aside from their own corporate email. Leasing/Invoices/Support/etc. Also please indicate if they need to have read only /read/respond as access.

  • End User 365 Licenses:

    Does this end user need just a Office 365 Business Email or the full Suite of 365 Apps (Word/Excel/PowerPoint/OneNote/Teams)?
